An agency sent my form back — what do I do?

Email support@namechangeworks.com from the address you bought with. This is the ordinary case, not a disaster: every form in the kit is checked by the agency that receives it, which is the whole reason a mistake surfaces as a returned application rather than as a problem years later.

What to tell us

1. Which form it was — the SS-5, your state’s license application, a passport form.
2. What they said, as close to word for word as you can manage. A returned form usually comes with a checked box or a line of explanation, and that line is the useful part.
3. Anything they handed you: a rejection notice, a stamped page, a letter.
4. Whether anything has changed since you answered the interview — a new address, a different mailing address, a name spelled differently on your document than you expected.

What we do

We prepare the form again from your answers and send you the new version. If the reason points to something we filled in wrong, that gets corrected. If it points to a detail only you know, we ask you and then re-run it. Either way there is no charge, and if you would rather have your money back than a second attempt, ask for that instead.

Agencies reject applications for many reasons, most of which have nothing to do with the paperwork itself — a document left at home, a certified copy they would not accept, records that had not caught up yet. Send us the wording anyway. It is the fastest way to tell which kind it was.
