Which forms do you fill in for me?
The SS-5 always, your state license form where your state publishes one we can fill, and clear instructions plus the official link for everything else.
Filled in for you
- Form SS-5, the Social Security application. It is the first step in every name change and it is free to file. We fill it on the agency’s own PDF, so what you print is the official form.
- Your state’s license or ID application, in the states that publish a form we can fill. Where yours does, the kit says so, and the filled form is in your downloads next to the state sheet.
Named, linked and explained
- The passport. Which of the three forms applies depends on when your passport was issued and how old you were — the kit works that out from your answers, names the one that applies, and links to it. One of the three is free if your dates qualify.
- The state license, where your state runs the change at a counter without a published form. You get the agency, the document list, the fee where we have verified one, and the link.
- The IRS, your voter registration and the Postal Service. There is less to do at all three than people expect, and the kit says exactly what.
Every form in the kit is the agency’s own document. We write your answers into it and nothing else — no stamp, no footer, nothing identifying us. The clerk who receives it sees the form they published.
Where a state does not publish a fee or a document list on its own pages, we leave that blank and say so rather than print a plausible number.
