Name Change Works for physicians
Coordinate Social Security, state ID, medical license, DEA registration, NPI, passport, and workplace records in a clear order.
Where a name mismatch shows up at work
- A physician’s name appears across the state license, DEA registration, NPI record, prescribing systems, payer enrollment, hospital credentialing, and patient-facing directories. Those records do not move together.
- NPPES places an explicit clock on the NPI record. Covered providers must report a name change within 30 days, and a name change can require additional verification documentation.
- DEA treats a registration name change as a modification, not a new base registration. A separate CSOS certificate has its own revocation and reissue process when that certificate is used.
- The Texas Medical Board says physicians must professionally use the licensed name and requires an application, written statement, and notarized supporting-document copy for the board record.
- No official source checked established a universal order among state licensure, DEA, and NPI. The page shows each sourced requirement without inventing a dependency.
The board or record you also have to update
State medical board, DEA registration, and NPI/NPPES
As one state example, the Texas Medical Board requires its Application for Name Change, a written statement explaining the request, and a notarized copy of the document granting the change.
DEA directs a registrant to modify the existing registration through Make Changes to My DEA Registration. Its official page says a new registration number is not required for a last-name change.
NPI changes are made with Form CMS-10114 through NPPES, on paper, or through an approved electronic file organization. CMS states that covered providers must report the change within 30 days.
Board certification is not treated as a settled procedure here, because we found no ABMS-wide or member-board process published; ask your certifying board.

What changes with Name Change Works
- Ordered checklist
Keep the federal identity sequence distinct from medical-board, DEA, NPI, payer, and hospital records, with the verified NPI deadline visible.
- Filled SS-5
Begin the government sequence with a filled SS-5 while completing the Social Security number box yourself after download.
- State form or state sheet
Use the state sheet to handle driver license or state ID requirements that can support identity checks in credentialing systems.
- Notification letters
Prepare a letter for each hospital, payer, practice, vendor, or licensing board you name. File the board application, DEA modification, and NPI update separately where required.
- Passport sheet
Keep passport work alongside conference travel and credentialing tasks, using the sheet to identify the form that fits the passport already held.
Your first hour
- Answer the interview
Finish the interview with identity, state, passport, and organization details, naming the practice, hospitals, payers, and professional offices that should receive letters.
- Buy and download your kit
Download the digital kit from the kit page and mark the 30-day NPI item before turning to slower or less clearly timed professional records.
- Start with Social Security
Prepare Social Security first and follow the kit’s state identification instructions so the government records move in a deliberate sequence.
- Handle the professional record
Open the state-board, DEA, and NPPES sources separately. Submit each required application or modification rather than treating a general notification letter as the filing.
What the kit costs
One payment includes the full digital kit: forms, letters, state guidance, and the ordered checklist. Digital is $49.
The digital kit is the purchase option currently shown on the pricing page.
If an agency bounces a form from the kit, Name Change Works will re-do it free for one year after purchase.
Questions and answers
How quickly must a physician change an NPI record?
CMS says covered providers must report an NPI name change within 30 days. Name changes can require additional verification documentation.
Do I need a new DEA registration number for a last-name change?
DEA says no. Use its registration-modification route. A CSOS certificate, if used, has a separate revocation and reissue process.
Does the kit file medical-board or NPI forms for me?
No. The kit organizes the sequence and can prepare notification letters, but you submit each board, DEA, or NPI request through the responsible office.
Is there a verified nationwide medical-board procedure?
No nationwide state-board procedure was established. The state example on this page is Texas, and other state-board instructions need to be checked directly.
