Name Change Works for nurses
Organize Social Security, state license, ID, passport, and workplace name updates with a filled SS-5, letters, and an ordered checklist.
Where a name mismatch shows up at work
- Your license name follows you through scheduling, onboarding, credentialing, and every verification request. A mismatch can send a routine renewal or employer check into a manual follow-up.
- Nursys does not take a name-change request from you. It displays information supplied by participating boards, so the issuing board remains the place to correct the professional record.
- A compact or multistate license adds another reason to identify the board that owns the primary record. NCSBN does not publish one national form, fee, or deadline for every nurse.
- Hospital badges, payroll, benefits, email, and clinical directories may sit in separate systems. Updating one workplace screen does not establish that the state license record changed with it.
- The useful starting point is a government-first sequence plus a separate board task, with the supporting document and license number ready before opening the board portal or form.
The board or record you also have to update
Your state board of nursing and its Nursys record
Contact the board that issued the nursing license. Nursys tells nurses to make corrections through the board because the board supplies the database record.
Use the state board procedure for the name shown on the license. NCSBN does not publish a universal application, submission channel, fee, or deadline that applies across all boards.
After the board processes the change, its data can flow to Nursys. A direct Nursys request is not a substitute for the board filing.
No state examples are published here, because individual board procedures have not been confirmed against those boards’ own pages.
What to have ready
- The supporting document required by the nurse’s own state board
- The nursing license number and the former and current names on the board record

What changes with Name Change Works
- Ordered checklist
Keep Social Security, your driver license or state ID, passport, employer records, and the board request in one sequence instead of treating them as one update.
- Filled SS-5
Start with a filled Social Security form, with the Social Security number field left blank for you to complete by hand.
- State form or state sheet
Use the state license and ID sheet to see the government ID step that often supplies the new photo identification requested elsewhere.
- Notification letters
Generate a letter for any hospital, staffing company, insurer, or licensing board you name. The board’s own form still has to be filed when that board requires one.
- Passport sheet
Keep the passport route beside credentialing tasks so travel identification is not lost among license, payroll, and employer changes.
Your first hour
- Answer the interview
Answer the interview questions about your names, state, identification, passport, and the organizations that need a notification.
- Buy and download your kit
Open the digital kit and put the ordered checklist at the front of the work, with your filled SS-5 ready for signature and the blank Social Security number field completed by you.
- Start with Social Security
Submit the Social Security change before working through state identification, then follow the timing instructions printed for your state.
- Handle the professional record
Open your state board’s official page and complete its separate request. Use a generated board letter only as a cover note when useful, never in place of a required board form.
What the kit costs
One payment includes the full digital kit: forms, letters, state guidance, and the ordered checklist. Digital is $49.
The digital kit is the purchase option currently shown on the pricing page.
If an agency bounces a form from the kit, Name Change Works will re-do it free for one year after purchase.
Questions and answers
Can I change my nursing license through Nursys?
No direct Nursys name-change process was published. Nursys says the boards of nursing provide its information, so contact the board that issued your license.
Does every state board use the same nursing form?
No uniform national form was verified. The form, channel, fee, and deadline are controlled by the individual state board and need to be checked on its official site.
Can a licensing board receive one of my notification letters?
Yes. Add the board by name and the who-to-notify step can produce a letter for it. If the board requires its own application or portal request, file that separately.
What happens if an agency bounces a form from my kit?
Name Change Works will re-do a form that an agency bounces, free, for one year after purchase. Tell us what the agency rejected so the form can be generated again.
