Name Change Works for attorneys
Coordinate Social Security, state ID, bar registration, PACER, passport, employer, and client-facing records in one sequence.
Where a name mismatch shows up at work
- An attorney name can appear on the state-bar roll, firm systems, court admissions, PACER, CM/ECF filings, trust and payroll records, email, and documents sent to clients.
- California’s bar form requires four identity documents in total: two showing the former name and two showing the new name, with a listed document sometimes serving in both groups.
- New York sends a name change back to the Appellate Division where the attorney was admitted. The statewide registration unit accepts the change after a written Appellate Division order.
- PACER personal information is changed through Manage My Account and routed to courts where the attorney is registered. Individual courts can also impose case-filing or documentation steps.
- The result is a set of controlled records, not one profile. Each bar, court, employer, and client-facing system needs to be identified and handled on its own terms.
The board or record you also have to update
Your state bar or admitting court, plus federal court accounts
California uses the State Bar Name Change Form. It requires two identity documents showing the former name and two showing the new name, for four documents total, and accepts mail, fax, or email.
New York requires a Name Change Affidavit and supporting material to the Appellate Division where the attorney was admitted. The statewide registration record follows the Division’s written order.
PACER directs the account holder to Manage My Account, open the Maintenance tab, and update personal information. The request is routed to courts where the attorney is registered for approval.
Federal court filing duties can differ by court and by open case. The Southern District of Florida source is one local example, not a nationwide deadline.

- California: four identity documents total, split between former-name and current-name evidence

What changes with Name Change Works
- Ordered checklist
Keep Social Security and state identification ahead of bar, court, employer, banking, and client-facing changes while preserving a separate task for each admitting jurisdiction.
- Filled SS-5
Prepare the Social Security filing from a completed SS-5, then add the Social Security number yourself in the blank field before submission.
- State form or state sheet
Use the state identification sheet to prepare the new ID that can become one item in a bar’s identity-document set.
- Notification letters
Generate letters for any firm, bar office, court clerk, bank, or insurer you name. A bar form, affidavit, PACER request, or case notice remains a separate filing where required.
- Passport sheet
Place the passport route alongside court-travel and identity tasks without treating a bar update as a substitute for a federal document change.
Your first hour
- Answer the interview
Answer the interview questions and list the firm, state-bar office, court accounts, financial institutions, and other organizations that should receive a direct notification.
- Buy and download your kit
Download the kit from the kit page and separate universal government steps from bar admissions and court-specific duties before preparing any jurisdictional submission.
- Start with Social Security
Start with Social Security, then follow the state identification sheet so new photo identification is ready for records that request it.
- Handle the professional record
Open each official bar or court procedure. Assemble California’s four documents, send New York material to the admitting Division, or enter the PACER account request as applicable.
What the kit costs
One payment includes the full digital kit: forms, letters, state guidance, and the ordered checklist. Digital is $49.
The digital kit is the purchase option currently shown on the pricing page.
If an agency bounces a form from the kit, Name Change Works will re-do it free for one year after purchase.
Questions and answers
How many identity documents does the California bar form require?
Four in total: two showing the former name and two showing the new name. The form explains when one listed document may serve for both groups.
Who changes an attorney name in New York?
The Appellate Division where the attorney was admitted authorizes the change. The statewide Attorney Registration Unit accepts the new name after the written order.
Does a PACER profile change reach every court automatically?
PACER says the request routes to courts where the attorney is registered for approval. A court may still require its own case notice or supporting document.
Can Name Change Works submit a bar or court request?
No. The kit organizes the work and prepares requested notification letters. You file the bar form, affidavit, account request, or case notice with the responsible office.
