Which states get a pre-filled form, and what the rest get
See whether your state receives a filled license form or a detailed state sheet, with the agency, requirements, fees, and official links.
State coverage, in plain words
Every state and the District of Columbia receives a license and ID sheet. It names the agency that handles the record, the official starting page, the documents the agency publishes, and whether the request is described as in person, by mail, or dependent on the transaction. State information is kept separate because one national instruction would hide real differences.
The deeper state records also include published fees, reporting windows, appointment details, and practical notes. Each dated fact comes from the state material recorded for that jurisdiction. If a fee, deadline, or requirement has not been verified, the sheet leaves the fact open and directs you to the agency rather than making one state sound like another.
A smaller set also receives the state's own application with interview answers filled into it. That set is determined by working form support, not by state population or by a claim that every agency uses a downloadable PDF. The current count and exact form numbers below come from the same coverage list used by document generation.
South Carolina needs two forms together, so both numbers appear. Wisconsin uses a different application for a driver license and for a state ID, and the kit selects the one that matches the credential answer. Hawaii publishes an application title without a short form number, so the official title is used instead of inventing an abbreviation.
States outside the pre-filled set do not receive a fabricated application. They receive the state sheet, the agency link, the bring list, and a direct statement where a fact remains unverified. Some agencies conduct the request entirely at a counter, and some publish flat or individually issued forms that are not suitable for filling ahead of time.
All state paperwork follows the same privacy and signature rules. Social Security number fields remain empty because the interview never collects the number. Signature and signature-date lines remain empty for you. When a state requires signing before a representative or a notary, the callout says so before you make the trip.

How it works in three steps
- 01
Choose your state
Your address selects the state coverage record, including the named license agency, official page, verified requirements, and supported form list. State-specific questions appear only when that state's application needs them.
- 02
Get the supported state artifact
Every kit receives a state sheet. When the coverage list and your credential support a fillable application, the matching official form is added with your answers already placed in its fields.
- 03
Follow the state's current route
Use the sheet to prepare the documents and personal marks, then confirm time-sensitive details on the linked agency page before visiting or mailing anything. The agency remains the authority for its current procedure.

Questions and answers
Does every state receive a pre-filled license form?
No. Every state and the District of Columbia receive a state sheet. A pre-filled application is added only for states and credential types in the supported form list shown on this page.
Why do some states receive instructions instead of a form?
Some agencies do not publish a downloadable name-change form, use a counter process, issue an individual form, or publish a document that cannot be filled reliably. The state sheet names the official route without substituting invented paperwork.
Where do the state form numbers come from?
They are read from the same state coverage records used to decide which official applications document generation supports. States that require two forms show both, and a form without a published number keeps its official title.
How current are the state details?
The page carries a checked date, and state sheets identify dated facts from the agency material used for that jurisdiction. The linked agency page is the place to confirm details that may have changed before you travel or mail documents.
