Your SS-5 and state form, already filled in
Get the official Social Security application and the state form your answers call for, filled in with clear notes for the few boxes you complete.
Pre-filled forms, in plain words
Your Social Security paperwork arrives on Form SS-5, the agency's own application, with the details from your interview already placed in the matching boxes. It is a form you can print and take or mail to Social Security, not a substitute document that asks a clerk to translate our layout into theirs.
If your state is in the current pre-filled set and the card you hold is one its form covers, your kit also contains the license or ID application used there. A form that excludes your card, as New York’s MV-44NC excludes REAL ID and Enhanced cards, is left out on purpose, and the state sheet says which application to use instead. Some states need two forms for one request, and the kit includes both when that is what the agency publishes. The exact state list and form numbers below are read from the same coverage data that decides which documents the kit builds.
If your state does not have a form in that set, you still receive a state sheet. It names the license agency, links to the official starting page, lists what to bring, and includes a fee or deadline when one has been verified from the state's own material. A missing form is not replaced with paperwork the state did not publish.
We never ask for your Social Security number. Any Social Security number box stays empty, and the kit names the exact box so you can write it in after printing. That keeps the nine digits out of the interview, stored data, downloads generated from stored answers, and notification letters.
Signatures and dates beside signatures also stay for your pen. A callout tells you where to sign and whether the signature belongs at home, before a notary, or in front of an agency representative. Everything else the form can take from your answers is completed before it reaches you.
Passport paperwork is handled differently because the correct federal form depends on dates and on the passport you hold. The kit gives you a passport sheet that identifies DS-5504, DS-82, or DS-11 and links to the official form.

How it works in three steps
- 01
Answer once
Tell the interview the names, dates, address, identification details, and state-specific choices that the supported forms ask for. A choice such as voter registration or organ donor status starts unanswered, so the form reflects what you selected rather than a default.
- 02
Receive the matching paperwork
The kit places those answers on the official SS-5 and, where supported for your state and credential, on the matching license or ID form. The state sheet sits beside it with the agency instructions and the list of documents to take.
- 03
Finish the personal marks
Print the form, add your Social Security number only where the form asks for it, then sign and date it where the callouts direct you. Send or carry the form by the method named in your checklist.

Questions and answers
Is the SS-5 an official government form?
Yes. The kit uses the Social Security Administration PDF and places your answers into its fields. The instruction pages remain with it, and you add your signature, signature date, and Social Security number after printing.
Will I receive a pre-filled license form in every state?
No. You receive one where the state publishes a form that the kit currently supports. In every other state and the District of Columbia, the state sheet names the agency, the requirements we have verified, and the official place to begin.
Do you collect my Social Security number for the SS-5?
No. The interview never asks for it. The number box stays empty on every generated form, and a callout names the box for you to complete after you print the document.
What happens if an agency sends a form back?
Tell us which form came back and what the agency said. We will prepare that form again from your answers at no charge for one year after your purchase.
