Read off the agency’s own pages. The date beside each row is the day we last checked it — a government fee with no date on it is worth very little.

What to bring to the Hawaii counter
- An original or certified copy of your marriage certificate, decree or court order — Hawaii names it a connecting document, and says notarized copies and faxes are not accepted
- If your document is a court order, Honolulu accepts one for a name change, adoption or divorce and no others — and it has to show your legal name, your date of birth and the court seal
- The document that establishes your legal name and date of birth: a US state or local government certificate or record of birth, a US passport or passport card, a Consular Report of Birth Abroad, a Certificate of Citizenship, or a Certificate of Naturalization
- Connecting documents establishing the link between the names on any documents that print different names

The sequence that works in Hawaii
Almost every failed name change is a sequencing failure — somebody at the counter before the records caught up.
- Do Social Security first — it’s the record most agencies check against, so getting it done first is what keeps the rest in step. A few states require it outright and say so on their own page.
- Apply at Hawaii — county driver licensing once Social Security has updated, which usually takes a day or two.
- Update your passport. Which form applies depends on when yours was issued.
- Update your vehicle title and registration, usually at the same counter.
- Update your voter registration, then your bank, employer, insurers and utilities.
What catches people out in Hawaii
- Hawaii licenses drivers at county level. The state DOT publishes the rules; your county DMV office does the work and sets the fee, so go to the county rather than looking for a state office. The DOT’s driver licensing page lists the office for each of the four counties.
- On Oʻahu the visit is by appointment only, through the City and County of Honolulu’s AlohaQ booking system at alohaq.honolulu.gov. Honolulu’s own name-change steps run: make an AlohaQ appointment at a Driver Licensing Center, complete the Driver’s License Application Form, submit proof of the name change, then eye test and fingerprinting, then payment of fees and a photograph. Steps in the other three counties may differ in detail; check your county’s page before you go.
- The application form is the same statewide — the State of Hawaii Driver’s License Application — even though the counties issue the cards.
- Any change to information printed on the card has to be proved in person, even for a US citizen or permanent resident who would otherwise be exempt from producing documents at renewal.
- Hawaii’s term for the paperwork is "connecting documents": where the names on your documents differ, you supply the ones that establish the link between them.
- A middle name or suffix is not treated as part of the legal name on Social Security documents — it can be abbreviated, omitted or misspelled without being a mismatch.
- A duplicate license can be applied for by mail if you are a citizen or permanent resident, but that route does not cover a name change: the proof has to be presented in person.
- Page 2 of the application is a voter registration application, and Hawaii will not process the license application until it is completed one way or the other — its own words are that the application "shall not be processed until you complete the voter registration portion of this application (see page 2) and indicate your choice of whether to register to vote". Registering, updating and declining are all choices you made in the interview, and your answers are printed there. What is left on that page is the signature and the date beside it.

Where your document comes from in Hawaii
Every agency in the sequence wants the original or a certified copy. Where you get one depends on which document you hold, and it is one of the few things that genuinely differs from state to state.
| What | Value | Source | Checked |
|---|---|---|---|
| Marriage certificate | From the state vital records office Hawaii charges $10.00 for the first certified copy of a marriage certificate. Orders can be placed online or by mail to the Honolulu office. | Hawaii Department of Health, Office of Health Status Monitoring | |
| Cost of a copy | $10.00 Each additional copy of the same record ordered at the same time is $4.00, plus a small administration fee. | Hawaii Department of Health, Office of Health Status Monitoring | |
| Decree or court order | Family Court, via the clerk Hawaii's Family Court handles divorce cases and its records are searchable through the state judiciary's case system. | Hawaii courts |
If you married or divorced in a different state, order from that state rather than this one. The record sits where the event was filed, not where you live now.
Whichever way your name changed
The Hawaii paperwork is the same three steps whatever authorized the change. What differs is which document you take to the counter.
Changing your name after marriage
Take your marriage certificate to Hawaii — county driver licensing.
- In Hawaii, a certified marriage certificate comes from the state vital records office — Hawaii Department of Health, Office of Health Status Monitoring.
- The Hawaii state office charges $10.00 for one certified copy.
- Hawaii charges $10.00 for the first certified copy of a marriage certificate. Orders can be placed online or by mail to the Honolulu office.
Changing your name after divorce
Take your divorce decree to Hawaii — county driver licensing.
- A divorce decree only changes your name if it contains a clause naming a former name and restoring it to you. Ending the marriage does not do it on its own.
- If your decree has no restoration clause, a counter clerk will refuse it, and the route back to a former name is a court petition instead.
- Agencies usually want the complete decree rather than the page carrying the clause, because the caption and the judge’s signature are on other pages.
Changing your name with a court order
Take your signed court order to Hawaii — county driver licensing.
- The copy that agencies accept is the one signed by the judge and stamped or sealed by the clerk. An unsigned conformed copy is refused.
- Agencies accept a signed court order on its own — none of them asks for a marriage or divorce document alongside it.
- The reason a court granted the order does not change the paperwork that follows it. Every agency downstream asks only to see the order.
Where a path has its own Hawaii page, the link goes there. Where it does not, it goes to the guide for that path, which holds in every state.
Hawaii name change questions
